Send a notification for an AD account that has not been created.

Is it possible to detect that an AD account has not been created due to a provisioning error (here due to a homonymy of email addresses) and send a notification?

Any provisioning account status is stored in reporting collections with the error message if the status is failed : Reporting Object Configurations

You can use an object policy to send the notification based on the collection: Object Policies
